A secondary condition branches off an island you already reached. Your service-connected condition caused a new one, and that new one is claimable too.
A secondary claim links a new condition to one you are already rated for. Common examples: PTSD leading to sleep apnea, a back injury leading to knee problems, PTSD leading to hypertension.
This is the key. You need a medical opinion stating the secondary condition is at least as likely as not caused or aggravated by your service-connected condition.
On the claim form, mark the new condition as secondary to your existing service-connected condition, and attach the nexus opinion.
